My Work

 
         My background is a Registered Nurse and had worked under Vancouver Coastal Health for nearly twelve years. I was a Licensed Practical Nurse since 2006 and then went to Athabasca University to finish my Bachelor of Nursing. I became a Registered nurse in 2011. During the twelve years of nursing, I had worked as a bedside nurse in many different units such as the spine, medical, neurology, cardiac and specialized dementia ward. I also had the opportunity to work as a temporary Patient Care Coordinator (PCC) for half a year.  After trying out the role of PCC, I realized I really enjoy leadership roles therefore recently I had changed my career path and got hired as a permanent Care Management Leader.
 

         My current role is to lead the iCare and ITH (Ideal Transition Home) by working together with the whole interdisciplinary health team.  The video below illustrates what I do daily by defining iCare and ITH. My job is to ensure patients' discharges are well planned to minimize readmission. Every day I work with different health professionals to ensure patients receive quality care while they are in the hospital and once they are discharged out into the community. I host the iCare rounds daily to get the most up to date patient information and base from that knowledge, I will draw a plan for the patient’s discharge date and the care that's required once they are discharged back into the community. My role links me to a variety of health care teams and facilities. This opportunity does not only widen my connections to different professionals but also broadens my knowledge of how each health care group functions.

 

         I love working in the health care field. Someday I would like to get involve in researches and work in the management sector to widen my knowledge.
